At Aluminum Jesus’ last concert in their hometown of Boston, rock singer Raeburn wrongfully accuses Spencer, a crew-member, of making a mistake. He verbally abuses and then attacks Spencer. Moments later Raeburn pours beer into the autograph book of a teenage fan. Bunny, Raeburn's disappointed girlfriend witnesses his outbursts. Boston Globe editor, Stevie Vanilla, also witnesses these outbursts and publishes the damaging story the next day. Raeburn has Spencer fired because he has the power to do so.
Gabriel, Aluminum Jesus' biggest fan, and his supportive wife, Isabelle, attend the show. Gabriel is excited to see them one last time. He's got leukemia and has only months to live. They enjoy the concert. Gabriel appears remarkably strong, unfortunately, after the show, Gabriel collapses.
While Gabriel recovers in the hospital, Isabelle calls a "Wish Foundation" and arranges for Gabriel to meet his idol, Raeburn. Bunny, who is business savvy, agrees to the meeting seeing this as a chance for Raeburn to redeem his bad behavior. Bunny offers Vanilla an exclusive on the story.
To complicate matters, Isabelle is pregnant and is afraid to tell Gabriel because she feels it may accelerate his death. She tells him anyway and Gabriel accepts the news. He wants to be strong for Isabelle.
Raeburn reluctantly meets Gabriel at the hospital, but soon they begin to bond and Vanilla is there to capture the meeting. It's good press. We learn that Raeburn and Vanilla have had bad blood in the past - Raeburn had sex with Vanilla's underage daughter. Vanilla and Raeburn patch their differences and Vanilla explains to Raeburn that he is like a suicide flower - those little flowers that grow between the cracks of sidewalks - and that Raeburn lives a fragile life, one step away from being trampled under foot.
There is an odd resemblance between the two men. Raeburn and Gabriel are both dying, in their own ways - Gabriel is the literal sense; Raeburn metaphorically. Raeburn feels that he is at the end of his career but seeing the resolve in dying Gabriel in the face of such tragedy, realizes that he has wrongfully fired Spencer and calls him many times, but Spencer does not answer - he's committed suicide.
To Gabriel's delight, Raeburn has driven Vera, his Lamborghini, to the hospital and they go for a ride. During their escapade, both realize they have more in common and Raeburn realizes he needs to be accountable for his life and how he treats others, especially Bunny. For the first time he calls and tells her that he loves her. We learn that neither Raeburn or Gabriel's parents ever told them about their past.
As they drive over the Tobin Bridge, traffic is held up, and a state trooper informs them that a jumpers’ body is being fished out of the water. The trooper has an odd name and voice. As they drive away, Raeburn calls Spencer once more and the trooper answers. Raeburn believes he is responsible for Spencer's death and goes into shock, mashes on the accelerator, seizes the transmission and it bursts. A small fragment pierces Raeburn's skull as Gabriel struggles to take control of the Lamborghini.
Gabriel has a special picture of his dead father, Bill, which is torn in half. It usually hangs over the toilet to give him comfort when he gets sick. Now, it is in his hospital room, bedside. Raeburn's beloved mother, Kathy, comes to the hospital but her son, Raeburn Messiah, is not listed. However, Raeburn Dalton is listed - his birth name. To Kathy’s shock, another Dalton is listed - Gabriel Dalton. She realizes that both of her sons are in the hospital and goes home to retrieve a shoebox of mementos and pictures.
Kathy meets Gabriel through Bunny and Isabelle and shares Raeburn’s life with them. This is when Gabriel discovers the other half of his photograph and that Kathy is his birth mother - and Raeburn is his brother. Gabriel is angry and wishes he never learned of the news. Then, the doctor informs them that Raeburn is brain dead and Kathy must decide whether to keep him alive on “the machines” or to pull the plug
The next day, while Raeburn is on his deathbed, Kathy explains to Gabriel, through a flashback, how the photograph was taken, on a family trip to Martha’s Vineyard, and that the strain on the young couple took a toll and they were soon divorced. Each parent took a child, thinking it was best for the children, and Kathy carried great guilt not being able to tell Raeburn about the past.
Through Isabelle, Gabriel realizes it is a good thing that he now has a mother and brother, even if he is going to die. In an ironic twist, because Raeburn is a blood sibling, he saves Gabriel’s life with his bone marrow.
This is not revealed until the end during a memorial led by Kathy at the Aquinnah Cultural Center, where the photo was taken, and is attended by Kathy, Isabelle, Bunny and Vanilla. Just as Raeburn’s ashes are about to sail from a bluff onto the ocean, Gabriel appears. After the ceremony, even though Isabelle is not showing, an Indian shaman predicts that she will have two children, the first a boy and then a girl. Isabelle reveals they will name the baby Raeburn. Finally, though deceased, Raeburn’s new CD has gone triple platinum.
